I have a patient with horrible outbreaks of pemphigus. She needs a implant. Here tissue is fryable and I dare not lay a flap. Is there a recommended therapy to reduce the pemphigus that will allow me to place the implant?
One option would be to get a very good history on the duration of the outbreaks and the length of time between them. You may be able to do the procedure between outbreaks. Same deal when it comes to placing the healing abutments. To get a hold of the situation pharmacologically, 60-80 mg of prednisone by mouth taken first thing in the morning for 10 days can do the trick. Beware of contraindications (diabetes, other infections, TB status, etc.)and side effects of steroids (candidosis, sleep problems, acne). Prednisone causes immunosuppression and delayed healing so don't place the implant immediately after tx. Maybe wait a month. You don't need a steroid supplementation during the procedure because your tx course was less than 2 weeks and a functional HPA response returns in 30 days. If there are pemphigus lesions in other parts of the body, consult a dermatologist. She may need some mycophenolate to boot.
