Inadequate KG/attached tissue around an implant: How do you handle.
Sat, 06/05/2010 - 06:29 — Tom House DDS MD
I frequently place implants in sites with inadequate KG/attached tissue.
How do you handle this as a clincian? Do you augment prior to stage I, at stage I or at stage II.
What technique do you utilize? Alloderm, Free Gingival Graft (FGG) or a Connective Tissue Graft (CTG)?

Inadequate KG/attached tissue around an implant: How to handle
Tom
Great question. I myself use a free gingival graft and I will usually do this at stage 2.
If it is in the maxilla sometimes you can grab more of the palatal tissue and apically position it around the healing abutement to get the desired KG.

Inadequate KG/Attached Tissue around Implant: What do you do?
I myself feel a free gingival graft is best. I usually do this at stage II and use a paquette knife to harvest the FGG.
I put a hole through the FGG graft and place the healing abutement right through the graft and stabilize with sutures.
My restorative dentist can take impressions can usually be taken in 6-8 weeks.
