Block Allograft: What determines the successful outcome?

I have done a number of block allografts with various success. I tend to favor zimmer blocks. I find they work great in the anterior but have poor results in the posterior. Im wondering is people have similiar experiences as me and what do you think determines a successful outcome with the block allograft?

In my hands for block allografts I like to:
1) Inlay the block into the site
2) Primary wound closure that is passive
3) I use growth factor such as prgf or gem21
4) two point fixation
5) no sharp corners
6) keep my fingers crossed
Block allografts: very technique sensitive not predictable
Block allografts are not very predictable. You can do everything write and they still dont work. You need passive primary closure and tend to work better premolar foward.
Inlay them in and use PRP.
predict your "block graft" everytime.
Regenaform in place of block graft, no need for second surgery. Cytoplast dPTFE barrier membrane, don't need primary closure. Completely predictable, if performed correctly, every time.
Block Allografts are good for width issues, not height
I learned early that block allografts are good for augmenting width but not for height. If you need height then you need an autogenous block.
I have stuck with this rule and found good success. Cardinal rules that I have followed for success are decorticate and inlay the recipient bed until you get bleeding. Stabilize the block. Get passive primary closure. Select your patients properly.
Block Allograft: Determinants of Success
I tend to get great results from Pre-molar forward.
Success is determined by stability of your block, I tend to use an inlay prep technique and alway cover the block with a membrane.
I also favor the use of PRP with the block allograft.
The most important aspect is getting PASSIVE PRIMARY CLOSURE. If the flap opens, you can kiss your block allograft goodbye.
